For Veterans with combat related incidents and their families.

 

1.  Please fill out the perspective client form and send it to PHR main office.  

2.  PHR main office will contact you to discuss your needs and desires and possibly tour our facilities.

 

 

PHR office is open Monday-Thursday 9:00 am to 4:00 pm.
